Membership
Membership is part of the managed care program through which regular and continuous monitoring of elder’s health is done through a multi-disciplinary team. This is done through detailed evaluation of the health and functional status at the beginning of membership and implementation of customized care plan throughout the year. A Health Manager becomes the single point of contact for all health needs of the elder. We share the responsibility of taking care of the elder along with their family and friends.
